A medical examination clinic is not a small hospital laboratory. Hospital work arrives steadily; applicant medicals arrive in batches, because a placement agency books thirty or fifty people into the same morning. Sizing the equipment on a monthly average is the mistake that turns a busy Tuesday into a two-day backlog.
The number that actually sizes the room
Ask for the busiest single day, not the monthly total. A clinic running 1,200 CBCs a month over 22 working days averages 55 a day — which almost any analyzer clears in an hour. But if the agency sends 40 applicants on a Monday and 4 on a Thursday, the Monday is the specification. Everything below is sized on peak day, not average.Hematology: the CBC is the engine
Every applicant gets a CBC, so this is the instrument that sets your pace. The decision is loading, not throughput — at these volumes almost anything is fast enough, but an open-vial machine needs someone standing at it tube by tube, and during a batch that person is the bottleneck rather than the analyzer.
An autoloader is what buys back the operator. The BC-5380 holds 30 tubes and runs them unattended; the BC-6000 holds 50. The BC-700 is open-vial but integrates ESR into the same run, which removes a separate workflow if your panel needs it. Compared in full in the hematology comparison.

Clinical chemistry: count tests, not patients
This is where clinics under-buy. Chemistry throughput is quoted in tests per hour, not patients per hour — and one applicant panel is many tests. Forty applicants at eight analytes each is 320 tests before you have touched a repeat or a control.
BA-88A is semi-automatic and suits a low-volume or backup role. The BS-240 runs up to 200 photometric tests per hour; the BS-240Pro up to 240, or up to 400 with the ISE module for electrolytes. Full range in the chemistry comparison.
Serology: plate capacity is the batch limit
ELISA runs a plate at a time, so your ceiling is plates per run — and a batch of applicants either fits on the plate or waits for the next one. The model number is the plate capacity: the ADC ELISA 200 and ADC ELISA 400 differ mainly in how many plates they hold, which is exactly the constraint that bites on a batch day.

Chest X-ray: the bottleneck nobody plans for
Laboratory benches parallelise — you can run chemistry and hematology at once. The X-ray room does not. It is one applicant at a time, and on a batch morning it becomes the queue everything else waits behind.
The practical choice is fixed versus mobile. A PLD5900A or PLD6500E is a permanent room with faster patient turnover; the PLX5200 is mobile and suits a clinic that also runs outreach or company screening on site.
