Mindray Microbiology Systems (Philippines): Blood Culture, ID/AST and the Workflow Between Them

Microbiology is the one category where the instruments are not alternatives to each other. They are stages in a workflow, and the right question is not “which one” but “where is the bottleneck”. This guide covers the Mindray microbiology systems we distribute in the Philippines and how they fit together.

The workflow, in order

1. Detect — blood culture bottles are incubated, agitated and continuously monitored until one flags positive. That is the TDR-X series.
2. Standardize — the organism is suspended and adjusted to a defined density, because ID/AST results depend on getting the inoculum right. That is the TDR-Z200.
3. Dose — the standardized suspension is dispensed into the wells of a 96-format panel. That is the AP-100.
4. Identify and test susceptibility — the panel is read to give an organism identification and an antimicrobial susceptibility profile. That is the AF-300.

A laboratory that already has ID/AST but reviews blood cultures manually has a detection bottleneck. A laboratory with automated blood culture but manual inoculum preparation has a standardisation bottleneck. They are different purchases.

Capacity is not the same as daily throughput. Because bottles sit for a predefined culture period — five days on the TDR-X060 and TDR-X120 protocols — a 120-position instrument supports roughly 24 new specimens a day, not 120. Size the instrument on daily specimens multiplied by the culture period, not on the number of slots.

The TDR-X060 uses irreversible colorimetric detection with multi-calculation algorithms, an enclosed drawer design to reduce environmental interference, and a dual heating system (solid heating bath plus air heating bath), with independent agitation per bottle.

Identification and susceptibility: the AF-300

The AF-300 is a fully automated ID/AST analyzer holding up to 32 comprehensive ID/AST panels for simultaneous processing. It uses Mindray combo ID/AST cards and a five-wavelength optical detection system matched to the characteristic wavelengths of different biochemical reactions.

It reports recognized resistance phenotypes including MRSA, BORSA, MRCoNS, VISA, hVISA, VRSA, VRE, HLGR, CRE, CRAB and CRPAE. For a laboratory supporting an antimicrobial stewardship program, that reporting is usually the reason to automate ID/AST at all.

The two instruments people forget to budget for

Automating ID/AST without automating the steps before it moves the bottleneck rather than removing it.

• The TDR-Z200 measures turbidity of bacterial suspensions in McFarland units across a 0 to 6 range. Inoculum density drives ID/AST accuracy, and standardising it by eye is the least reproducible step in the whole chain.
• The AP-100 automates dosing of the suspension into 96-format plates using a syringe-and-pump mechanism with multi-position dispensing — removing the most repetitive manual step in the workflow.
